Up-to-date Immunization Records

· New Physical if out of state - A waiver in the registration packet can be signed and used until a new physician can be found for the physical.

· Original Birth Certificate

· Previous school address - (for transfer of records)

· School Records(Report Cards, Assessments)

· Copies of Individualized Education Plans for students receiving Special Education Services

· A Copy of the Purchase Agreement if the House is not completed before starting school
